How to complete the

CERTIFICATION OF BIOHAZARDS CONTROL FORM

Students planning research involving Biohazards MUST, before starting any experiments, COMPLY with the rules and guidelines of the International Science and Engineering Fair (ISEF).

 

For all research involving potential biohazards:

 

Become familiar with the International Science and Engineering Fair (ISEF) Rules and Guidelines, Potential Biological Hazards.

 

  1. Read and complete the Greater San Diego Science and Engineering Fair Certification of Biohazards Control Form (GSDSEF Biohazards Control Form). Include all required signatures.

 

  1. A teacher shall serve as advisor for the project in which case he/she assumes primary responsibility for the project. If there is no teacher, a Designated Supervisor or Adult Sponsor must provide certification that safety guidelines are followed. 

 

  1. In addition to the student signature, certain adult signatures may be required on the GSDSEF Vertebrate Animals Form depending on the location of the student's research. Additional forms may be required by ISEF depending on location of student research.

 

     Forms and Signatures Required--In addition to the GSDSEF Signature Page:

  • If research is conducted at a middle or high school:  
    • GSDSEF Biohazards Control Form signed by: 
      • Parent/Guardian
      • Teacher/Advisor

 

  • If research is conducted at a Regulated Research Institution:
    • GSDSEF Biohazards Control Form signed by:
      • Parent/Guardian
      • Teacher/Advisor
    • Supervising scientist signature on the GSDSEF Biohazards Control Form, ISEF Form 2 (Qualified Scientist) and if tissues are used: ISEF Form 6B (Human and Vertebrate Tissue Form)

 

  • If research is conducted at a middle or high school with scientist training/supervision:
    • GSDSEF Biohazards Control Form signed by:
      • Parent/Guardian
      • Teacher/Advisor
    • If another adult is trained: Designated Supervisor signature on the GSDSEF Biohazards Control Form
    • Qualified Scientist signature on the GSDSEF Biohazards Control Form and ISEF Form 2
    •  

 

  • Projects involving Biohazards are prohibited in the home environment.  However, for descriptions of projects involving living or dead biological organisms/material which are allowed—and do not require the GSDSEF Biohazards Form—see ISEF Rules and Guidelines: Potentially Hazardous Biological Agents (PHBA) Rules, and PHBA Risk Assessment.

Submit the GSDSEF Biohazards Control Form and any other additional forms necessary forms with the SRC Pre-Approval Form to the GSDSEF Scientific Review Committee (SRC) for approval.
